To receive reimbursement for any prescription medication(s) you paid out pocket for, you may submit your receipts or fill out a Prescription Reimbursement Form to receive applicable reimbursement. Reimbursement received may vary upon benefit guide lines.
You can print the Prescription Reimbursement Form, fill it out, and mail or fax it to SUNRx. Please allow up to 6 weeks for your reimbursement check to be issued.
